IMMUNIZATION RECORDS UPDATE

It's that time of Year Again! - Immunization Records Update

Region of Waterloo Public Health is required to collect and analyze immunization records of all children attending school in Waterloo Region. We do this to ensure the immunization status of all children in the event of a vaccine preventable disease outbreak.

The Immunization of School Pupils Act requires students to be immunized against tetanus, diphtheria, polio, measles, mumps and rubella. These records of immunization must be provided to Public Health. With appropriate legal documentation, exemptions based on medical or conscience reasons are allowed. If students do not provide the immunization record or legal exemption to Public Health, they may face suspension from school for up to 20 days or until the documentation is completed and received.

In early March 2010, Public Health will send a notice home to those students with incomplete records. This notice will request that complete immunization records be provided to Public Health. If you receive one of these letters, please respond as soon as you can.

If you have any questions, please contact our Vaccine Preventable Diseases Program at 519-883-2006 ext. 5273.

Thank you for your cooperation.
